CCPA ADDENDUM

INTRODUCTION
This Additional California Privacy Disclosures document supplements La Belle Medical Spa's Privacy Policy and is provided pursuant to the California Consumer Privacy Act of 2018 (CCPA) as amended by the California Privacy Rights Act of 2020 (CPRA). It applies solely to visitors, clients, and individuals who reside in the State of California.
Any terms defined in the CCPA have the same meaning when used in this Addendum.
1. CATEGORIES OF PERSONAL INFORMATION WE COLLECT
In the past 12 months, La Belle Medical Spa has collected the following categories of personal information:
Identifiers Name, email address, phone number, mailing address, IP address, and similar identifiers collected through our website contact forms, booking platform, and membership enrollment.
Customer Records Name, contact information, and appointment history collected through AestheticsPro, our electronic health records and scheduling platform.
Commercial Information Records of services purchased, treatments received, membership enrollment, packages purchased, and retail products purchased.
Internet or Electronic Network Activity Website browsing behavior, pages visited, time spent on site, and interaction with embedded content collected through website analytics tools.
Communications Data Text messages, emails, and chat messages exchanged through Podium and other communication platforms in connection with appointment scheduling and client service.
Inferences Preferences and service interests inferred from your appointment history and treatment choices to personalize your La Belle experience.
2. CATEGORIES OF PERSONAL INFORMATION WE DO NOT COLLECT
La Belle Medical Spa does not collect the following categories of personal information through our website or general client intake process:

Sensitive personal information such as race, ethnicity, immigration status, or religious beliefs

Financial information — credit card and financing data is collected and processed exclusively by third-party platforms including Cherry and is not stored by La Belle Medical Spa
3. PURPOSES FOR WHICH WE COLLECT PERSONAL INFORMATION
We collect personal information for the following business purposes:

4. SALE OR SHARING OF PERSONAL INFORMATION
La Belle Medical Spa does not sell your personal information to third parties. La Belle Medical Spa does not share your personal information with third parties for cross-context behavioral advertising purposes.
Because we do not sell or share personal information as defined under the CCPA, we do not offer a "Do Not Sell or Share" opt-out mechanism at this time. If this practice changes, we will update this Addendum and provide appropriate notice.
5. THIRD-PARTY PLATFORMS
La Belle Medical Spa uses the following third-party service providers who may receive certain personal information in connection with the services they provide to us:

RepeatMD — membership enrollment and loyalty program

Cherry — patient financing (financial data handled exclusively by Cherry)

Podium — client messaging and review management
Each of these platforms operates under its own privacy policy and CCPA compliance framework. La Belle Medical Spa does not control how these platforms collect, store, or use personal information beyond what is necessary to provide services to our clients.
6. RETENTION OF PERSONAL INFORMATION
La Belle Medical Spa retains personal information for the following periods:
Medical and Treatment Records Retained for a minimum of 7 years from the date of service in compliance with California medical recordkeeping requirements, or until the patient reaches age 19 for minors, whichever is later.
Appointment and Client Records Retained for the duration of the client relationship and for a reasonable period thereafter as required for legal and business purposes.
Marketing Contact Information Retained until you opt out of marketing communications. You may opt out at any time by emailing info@labellemedicalspa.com or by clicking the unsubscribe link in any marketing email you receive from us.
Website Analytics Data Retained in anonymized or aggregated form in accordance with our analytics platform's standard data retention practices.
7. YOUR CALIFORNIA PRIVACY RIGHTS
As a California resident, you have the following rights under the CCPA and CPRA:
Right to Know You have the right to request that we disclose the categories and specific pieces of personal information we have collected about you, the categories of sources from which it was collected, the business or commercial purposes for collecting it, and the categories of third parties with whom we share it.
Right to Delete You have the right to request deletion of personal information we have collected about you, subject to certain exceptions including where retention is required by law, such as medical recordkeeping obligations.
Right to Correct You have the right to request correction of inaccurate personal information we maintain about you.
Right to Opt Out of Sale or Sharing As noted in Section 4, La Belle Medical Spa does not sell or share personal information. This right is currently not applicable but will be honored if our practices change.
Right to Limit Use of Sensitive Personal Information La Belle Medical Spa does not collect sensitive personal information as defined under the CPRA through our website or general intake process.
Right to Non-Discrimination La Belle Medical Spa will not discriminate against you for exercising any of your CCPA or CPRA rights. Exercising your privacy rights will not affect the quality of care or services you receive at either of our locations.

Please include your full name, the location you have visited or the email address associated with your La Belle account, and the specific right you wish to exercise. We will respond to verified requests within 45 calendar days as required by the CCPA. If we require additional time, we will notify you within the initial 45-day period.
We may need to verify your identity before processing your request to protect the security of your personal information.
9. AUTHORIZED AGENTS
You may designate an authorized agent to submit a CCPA request on your behalf. To do so, the authorized agent must provide written permission signed by you, and we may require you to verify your identity directly with us before processing the request.

11. CHANGES TO THE ADDENDUM
We may update this CCPA Addendum periodically to reflect changes in our practices or applicable law. The "Last Updated" date at the top of this page will reflect any revisions. Continued use of our website or services after any update constitutes acceptance of the revised Addendum.
